Is there a special way I need to download this if I am currently playing on an older save file when uploading this?
Unfortunately not. If you want to transfer your household progress/families, you can basically save them to library and move them in this new save. They will keep the careers and skils. If you have a big family tree you can make a very large household (more than 8 of you use mods) and you can move the whole family tree in the updated save and them split them.
Does this save file use paid items from the marketplace?
I planned to not use anything from the marketplace, but I realised now I used by accident some items, I will update the save with those items replaced ASAP.
