The reason Narancia shot Risotto with his stand is mainly due to the scalpel being thrown at the group. They could prob tell exactly what direction it came from, and Risotto's breathing was the only breathing within range in that direction. They do risk killing someone innocent, but the chances of that were extremely low.
Risotto was literally seconds from killing the boss. Even though it was doppio at the time they share the same body however doppio is clearly way weaker than diavolo because he's not able to use the time powers of king crimson

Doppio is the boss, he's just a split personality of the boss. The boss shares a body with doppio. The full stand King Crimson is only available when the boss is in charge of the body, while Epitaph (the little face on the head), which is what allows King Crimson to see the future before erasing it is available to both the boss and doppio.
