Many were disappointed or questioned why sasuke didnt dodge borutoshiki's attack when his eye was stabbed and this is the exact reason.

The reason why kawaki was able to get through without getting detected is cuz hes an ohsutsuki. They are someone who cant be tracked with sensory ninjas. An ohsutsuki can only be tracked by another ohsutsuki. Thats the reason why only boruto was able to do it. This was established in the show a handful of times: 1) Naruto can generally track people countries away like in the war arc. But during the chunin exam he never sensed them until they came to the arena. Also, if you remember naruto felt that sensation not because of tracking but because of their "evil intent". In thr war arc, thats how naruto tracked white zetsus. 2) The reason why sasuke didnt dodge borushiki's kunai when hr stabbed his eye was because of this. He never sensed momoshiki moving towards him until he was stabbed in the eye. 3) During fights vs jigen and isshiki, many times naruto couldnt track his opponent at all. Sasuke was the one who was able to see jigen using his EMS. Thats why sasuke figured out jigen's ability to shrink.
