Scimitar of the Silver Naga
Weapon (scimitar), very rare (requires attunement)
This magic scimitar deals an extra 1d6 poison damage to any creature it hits. While attuned to it, you have resistance to poison damage and have advantage on saving throws against poison.
Incinerating Serpent. While holding the scimitar, you can use an action to speak its command word and create an instantaneous naga of pure light. Choose a space that you can see within 30 feet of you for the naga to begin in: it then instantly streaks forward from that space in a contiguous path, which is composed of up to ten 5-foot cubes. Each cube can be placed anywhere—vertically or horizontally—as long as it's adjacent to the previous one. Each creature within the naga's path must then make a DC 16 Constitution saving throw. On a failed save, a creature takes 8d6 radiant damage and is poisoned for 1 minute; while poisoned in this way, the creature is also blinded. On a successful save, a creature takes half as much radiant damage and isn't poisoned. A creature poisoned in this way can repeat the saving throw at the end of each of its turns, ending the effect on itself on a success. This property can't be used again until the next dawn.
Naga's Restoration. If you die while the scimitar is within 60 feet of you, you are immediately restored to life with half your maximum number of hit points and are cured of any conditions affecting you. The scimitar then loses this property as well as its Incinerating Serpent property.
The silver glint of pure radiance: when you stare into its purity, do you feel its cruel sting?

I feel like it needs a sentience that must sacrifice itself to prevent your death.
Super fun! I totally encourage that for your table!
