I'm thinking Pestilence, on his way to visit Death, after finding out she is stuck surrounded by clouds of very interesting poisin.
Horseman of pestilence
Asking to guess implies it is someone already named, but that does not fit any of the 6 non-horsemen grand leaders descriptions, and it feels like pestilence has been alone for to long for that infrastructure to still exist, though maybe he killed the bacteria as well, so my guess is Famine.
Hint. No one ever said this was a planet ruled by or controlled by the person. Or even that they had ever been to that planet before.

My guess is Famine, based on the backgrounds. I don't think Pestilence cares to surround himself with struggling neighbourhoods. Could, of course, also be Pestilence. But I'll stick to my first guess.
I envisioned it being pestilence on a run to destroy a planet or test a new disease hence the people in the background.

Rex is a world. Not a person. And no, Rex is a massively high tech sci-fi like world.
