Denys, I would also investigate the flight profiles of previous flights by these pilots to see if there is a pattern of similar approaches. Also, what are the company SOPs, and what checking or review of flight profiles does the airline carry out? Maybe the pilot had a medical issue, and the copilot lacks the confidence to take control and go around. There are many variables that could be causal factors,
airline hired them, airline is responsible. whatever punishment for pilots, much should be applied to airline.
If you as a pilot can look at the available data and reach the conclusion that the crash was caused by grievous pilot error, we can hope that NTSB with hands-on contact, and first person interviews with everyone concerned will be able to reach accurate conclusions. Thanks for sharing your expert insights.
Absolutely banging expert opinion and fact reporting. So important for somebody to at the very least ask the tough questions as the smoke is still rising. Then in the clear light of day, call it as it is. This hard-edged view positively affects and influences how seriously we take these accidents. For all those who make serious mistakes and errors, too often their culpability diminishes in proportion to how much time passes.
What kind of CRM was in that airplane?
