preg content is best content. i really do wonder how multiple pregnancies are going to work though, especially with all the mutators that increase the possible number of simultaneous offspring. i'm going to assume some body types (eg. 4'6" girls, narrow hips, frail bodies, etc.) have limits on how many children they can carry at once, as well as how long they can carry for.

Well... My main plan is to have Bad Things™ happen to a player that gets too pregnant for their specific body, and who don't have the benefit of advanced medical support. While outcomes will vary based on the organ donor setting, the general idea is that each pregnancy will be split into different ranges. Too many babies for the player's body (narrow hips, low health, etc) will result in a full or partial miscarriage pretty early, unless you're in a certain storyline. There's the safe area, which proceeds pretty normally as long as nothing happens. The interesting area is the grey-zone, where the player teeters on the edge of what their body can support and survive. Then the expected sorts of mechanics like bed rest, being healthy, and medical care will come into play to help tip the odds. Of course, I think that the normal game will probably be quite generous with how many babies are possible with and without risk.

I'm guessing that's either a planet/moon or some dirt on a finger/knee/toe.
