Why doesn't the returning back scene play with Steph in the car

There are a few reasons for this. Did you get infected by Tanya (if you got her club bathroom scene)? If so, you can't access this scene, you need to replay it without interacting with Tanya as you're waiting to go to the hospital before interacting with Steph again at the time the scene otherwise occurs.
