Some lore about the chinese dynasty at the time based on what i understood: The concubine who births a male heir to the emperor, if the emperor marries her, becomes the emperess, until their male heir marries another woman who she then becomes the emperess, and the emperess (mother of the heir) would relinquish her title of emperess to the new woman, and she becomes the emperess dowager. The emperess (old woman who died, mother of the former emperor) was called the emperess instead of emperess dowager (which means no-longer emperess, but acknowledged as former emperess). It's not explained yet why that's the case. But because both the old emperess and the current emperess dowager co-existed, that's why they have separate titles. The former emperor's wife (current emperor's mother) was called emperess dowager because she retained less power than the emperess at the time who wielded more power in the hierarchy while the former emperor was alive (for reasons yet to be explained). So the fact that they called the old woman emperess instead of emperess dowager when the new woman (the young anshi) was introduced to the royalty line, implies that there's some bad shenanegans that happened around that time, where people didn't acknowledge her as the emperess, probably because she was married to the former emperor, again, to be explained later. But there's definitely some bad blood that they're seeding as exposition just with the names alone. So you're right to question things.
