No spoilers here, so feel safe to read. If we assume that the baby swap with Ah-Duo etc. etc. is legit. Then there are 2 layers to Jinshi's hidden identity. What he meant by "He needs to make himself a new prince" is as such: 1 - Jinshi the Eunuch is the top layer fake identity. 2 - Jinshi was raised as the swapped baby, so he lives as "the sickly, not often seen" little brother of the current Emperor. 3 - In truth he'd be the son of the now-emperor and Ah-Duo thou, thus also making him the sole living heir and prince currently. Should the emperor die prematurely, Jinshi would become emperor, as he's "the Emperors Little Brother" and next in line, as long as the Emperor has no male heir. (I get confused how aware Jinshi is of all the details too sometimes, while thinking about it, but this is the gist of it) All this allegedly, if we believe the conjecture different characters provided us with. Jinshi doesn't want the responsibilities of being Emperor or even the heir, thus his monologues about not being able to live freely etc. and him wanting to escape this, if the Emperor only had a publicly known proper heir.

The Empress Dowager is the Emperor's mother, not wife. Ah-Duo was with the current Emperor before he was the Emperor, back when he didn't have to have multiple concubines. If we are accepting that "Jinshi" was the swapped baby that lived, that would make him the son of Ah-Duo and the current Emperor, but he would be under the impression that he is the current Emperor's brother.

Jinshi and Gaoshun are drinking an anaphrodisiac, the opposite of an aphrodisiac, designed to suppress libido. Historically such medicines have been used either as treatments on sex offenders (also known as chemical castration) in the hopes that suppressing sexual urges solves the problem, or by religious sects who wish to rid themselves of "impure" thoughts and sins. They don't explicitly state what kind of anaphrodisiac they're taking, but Gaoshun hints that one of the side effects of prolonged use could be permanent infertility.

Consider it like he knows who he grew up to be, he knows who is pretending to be now, but doesn't know where he came from at his birth.
